Unfortunately currently booking appointments online is not practical. We need to ask many questions and appointment availability is too difficult to align through multiple email communications. This will ensure you get the appointment you need, when you need it as well as allowing us optimal control of our diary management.

We will need the following information:

• Name, address, Date of birth, telephone number.
• Type of appointment requested
  
  1. Sight test
  2. Contact lens check or new fitting
  3. Driving licence test (Group 1 or 2)
  4. Dispensing only appointment.
  5. Other
• Medical Card Holders – need to be pre-approved by the health board. We can now do this online with your medical card number.
• PRSI allowances can be confirmed with your PRSI number by our staff using welfare partners online portal.
