Unfortunately due to the current Covid 19 crisis booking appointments online is not practical. We need to ask many questions and appointment availability is too difficult to align through multiple email communications.

We will need the following information:

• Name, address, Date of birth, telephone number.
• Type of appointment requested

  2. Contact lens check or new fitting
  3. Driving licence test (Group 1 or 2)
  4. Dispensing only appointment.
  5. Other
• Medical Card Holders – need to be pre-approved by the health board.
• PRSI allowances can be confirmed with your PRSI number by our staff using welfare partners online portal.
• Covid questionnaire regarding your current covid health status and exposure risk.
